Vital Statistic InformationThe funeral director is required to obtain statistical information about the decedent and report it on the ORIGINAL DEATH CERTIFICATE to the Registrar of Vital Statistics in the municipality where the death occurred...the local registrar then forwards a copy of that information to the Registrar of Vital Statistics of the State of New Jersey.
CERTIFIED COPIES of the ORIGINAL death certificate can be purchased as legal proofs of death from either the local registrar or the State Registrar in Trenton.
The information obtained by the funeral director is to be the "best information available" from the informant of record and may be changed on the formal record via a set procedure by anyone of authority who developes more accurate information in the future.
Some of the information that is required on the original death certificate is also reported to various newspapers in the form of an obituary. If any specific items are not to be reported to the newspapers, the informant should so specify at the time that information is given to and recorded by the funeral director. Below, italicized information is usually NOT released to the newspapers...(parenthesized information is specifically FOR newspaper releases.)
A list of the various statistics that are needed appears below (with sub-items).
Name of the decedent - first, middle, last, suffix, other name (nicknames)
Date of death
Place of death - name of facility or street address, town, county, state, zip code
If in a hospital, Inpatient? - ER/Outpatient? - Dead On Arrival?
If NOT in a hospital - Nursing home? or Residence? or other? specify.
Place of residence - street address, town, county, state, zip code (Length of time here and town of prior residences)
Marital status - Never Married or Married or Widowed or Divorced [Separated is not a valid response for this item]
Race
Sex
Highest formal school grade completed
Veteran of US Armed Forces? If yes, give dates of service
Was the decedent of hispanic origin? If so, specify which country.
Social Security Number or Railroad Retirement Number
Date of birth
State of birth - City and country, if known.
Citizenship
Age
Father's name
Mother's maiden name
Name (maiden) of surviving spouse - (Name of late spouse and year of death)
Occupation most of the decedent's working career ["Retired" status is immaterial] (Other occupations and employers)

Type of final disposition [burial, cremation, entombment or other]
Place of final disposition - name of the cemeteryor crematory or mausoleum , town, state

Additional information will be developed through the course of the Arrangement Conference at the funeral home between the funeral director and those present...the informant should have the following information available for distribution to the newspapers:
A list of survivors, their relationship to the decedent, their home towns and, if desired, spouse's names (children, brothers and sisters, parents, if surviving, grandchildren and anyone else to be listed in an obituary)
A list of organizations that the decedent was a member of along with any offices held or honors bestowed (religious, fraternal, civic, military, occupational, political etc.)
A list of any activities that the decedent enjoyed that should be released to the newspapers
The name and address of any charitable organization that the family would like to mention as a recipient of memorial contributions
This information need NOT be put in any specific format and the funeral director will compose the information in obituary format...and the obituary staff of the newspaper will probably change it again. :-(
It would also be helpful if the informant will bring the following items to the funeral home at the time of the arrangement conference.
The decedent's discharge from military service (the original or a photocopy)
The deed to the cemetery where burial will occur (the original or a photocopy)
A recent snapshot or photograph of the decedent (for the embalmer/hairdresser or to be released with the obituary)
Burial clothing (a full set of clothing, including underclothing and hose - shoes are not needed - any jewelry to be used - glasses if the decedent usually wore them - dentures)
